Health Net Grant to Support Tuolumne County Public Health

February 6, 2026

Health Net is giving over $22,000 to Tuolumne County Public Health to improve community health. This grant is part of our Population Health Management (PHM) Program, which focuses on better care, stronger data systems, and community partnerships. The funding will help fight local health challenges, including the opioid crisis.

What is Population Health Management?

PHM works to make healthcare fair, effective, and responsive. It aims to:

  • Build trust with members
  • Use data to guide care
  • Address root causes of poor health, like housing and food needs
  • Promote wellness and prevention
  • Provide special care for high-risk members
  • Ensure smooth care transitions
  • Reduce health disparities

This grant supports these goals by giving Tuolumne County tools to make better decisions and improve health for everyone.

Improving Data Systems

A big part of the grant will upgrade the county’s data systems. Good data helps public health leaders see trends, plan programs, and measure results. The grant will provide access to Manifest MedEx (MX), California’s health data network. MX offers:

  • Chronic Conditions Risk Report – Finds people at high risk for health problems so they can get help sooner.
  • Health Equity and SDOH Report – Shows data on race, language, and social needs to help reduce health gaps.
  • Planning Tools – Supports community health plans with accurate, timely data.

Building Community Partnerships

Public health also depends on strong community connections. The grant will fund meetings and events that bring people together to solve problems, including:

  • Quarterly Opioid Coalition Meetings – Key to reducing opioid harm.
  • Medical Society Functions – Aligns doctors with public health goals.
  • Tuolumne County Resiliency Summit – Focuses on building a stronger, healthier community.

Expected Benefits

This investment will:

  • Improve decision-making with better data
  • Train staff for long-term success
  • Strengthen partnerships to tackle tough issues
  • Reduce health disparities
  • Help the county take a proactive approach to health

Health Net is proud to support Tuolumne County in creating lasting, positive change for its residents.
