EQuIP-LA is:
- A two-year improvement collaborative.
- Led by the California Quality Collaborative (CQC).
- Committed to advancing equitable healthcare and reducing care disparities faced by Medi-Cal enrollees of color.
Throughout this collaborative, providers and organizations have been supported in their mission to deliver higher-quality care through these key efforts:
- Empowering practice coaches and communities: Providing tools and resources to drive equitable quality improvement.
- Patient-centered learning: Enriching the collaborative curriculum with real-life patient experiences to shape better care outcomes.
- Focusing on chronic disease management: Developing strategic solutions to improve care quality in chronic disease management, with a focus on diabetes, hypertension, and colorectal cancer screening.
- Enhancing primary care skills: Strengthening abilities to deliver consistent high-quality primary care.
