Getting preventive care is not always easy. Many people and families have to manage appointments, find transportation, and take time off work. Because of this, important health screenings are often delayed.
At Health Net, we believe better community health starts with making care easier to reach. That is why, in 2025, Health Net gave a $50,000 grant to Heritage Provider Network partner Regal Medical Group. The grant funded community clinic day events in Los Angeles and Riverside counties.
Through this program, Regal Medical Group held five clinic days to bring care closer to where people live. These events offered many services in one place, including preventive screenings, telehealth visits, and in-home care. Instead of making many trips, members could get more than one service during a single visit.
The program reached more than 500 members and helped connect people who had not gotten preventive care in a while. In some cases, a routine visit became a chance to address health needs at once, from screenings to follow-up care. This made care easier to manage and saved time.
One main goal of the clinic days was to improve access to preventive screenings, including breast cancer screenings offered through onsite mobile mammography units. Telehealth visits and in-home care also helped members get support after the events. This gave them more ways to stay connected to care and address ongoing health needs.
One of the most important results was bringing care together in a way that worked better for members. By offering many services in one place and helping coordinate care, the program reduced common barriers such as transportation, scheduling, and follow-up needs. This made it better for people to take the next steps in their care.
Programs like this show a more connected approach to community health. The focus is on access, coordination, and meeting people where they are.
